As the Director of Home and Community Based Services (HCBS) you are responsible for coordinating and managing services for our residents in their homes.  You are also responsible for developing and growing additional community based opportunities beyond our campus.  In addition the Director will work with our Thrive at Home program, assisted living, and personal options program to provide support and coordinate services to residents and members in their homes.    This includes but is not limited to nursing services, aide services, companions and memory support.  

Position Responsibilities:

  • Will assist in the development of initiatives involving expansion of health services both on campus and throughout the greater New Haven area.
  • Oversees all aspects of health care on campus and off campus through our Thrive At Home program.   
  • Participates in or develops objectives, standards of care, policies and procedures concerning services delivered to residents.
  • Assists in staff recruitment, selection, orientation and in-service education.
  • Participates in the development of resident care plans - identifies resident needs, goals and plans for intervention.
  • Working with our Resident Resource Team, coordinates all services for residents, both on and off campus.
  • Provides oversight of the Assisted Living Services with the support of the Assisted Living Supervisor.

Education:

The right candidate for this position must have a bachelors degree from an accredited college or university.  

Professional Experience:
We are looking for an experiences leader who is excited about what is possible in the future of aging services.  A leader who has worked to improve existing programs and services.  Someone who has had a vision and worked to see it come to life.  Prior aging services and home and community based services experience is a plus.
